You know the routine: drive to the gym, change clothes, sit in the sauna for twenty minutes, shower, go home. No exercise happens. People call this the "executive workout," usually as a joke. Sometimes it's a phone call. Sometimes it's just quiet thinking time.
We won't pretend we're above it. Between long patient days and a busy week, one of us has done this more times than we'd like to admit.
Here's the honest truth about what those twenty minutes actually do. It's more than the joke suggests, but less than the wellness influencers claim. Heat shares one real benefit with cardio exercise. It also does something exercise can't do well on its own. Doing it in two rounds, with a cold plunge in between, does that job much better than one long sauna session.
That second benefit is what this article is really about.
The Problem Patients Describe
Many of our patients describe the same feeling. They feel heavy and thick, especially in the morning. Their hands and feet look puffy. Their rings fit differently from one day to the next. They're stiff for the first hour after waking up. They use words like "stagnant," "congested," or "waterlogged." Most can tell us roughly when it started. Almost none can tell us why.
This is a real, physical problem and it happens in a specific place. Not in your bloodstream. In the space between your blood vessels and your cells. Most standard checkups never look there.
Where the Fluid Actually Goes
Every cell in your body sits in a bath of fluid. This fluid isn't blood - blood stays inside your blood vessels. The fluid around your cells is called interstitial fluid, and it's how oxygen and nutrients get delivered. Oxygen leaves the blood vessel, crosses this fluid, and reaches the cell. Waste travels the opposite direction.
For a long time, doctors were taught that this fluid flows out of blood vessels on one end and flows back in on the other, like a simple loop. We now know that's not quite right. Fluid leaks out along almost the entire length of the blood vessel, and very little of it flows back in the same way. Once it's out, it doesn't go back through the same door.
It leaves through a different system entirely: the lymphatic system.
This matters because the lymphatic system has no central pump — nothing like the heart. Lymph fluid moves for four reasons: the lymph vessels squeeze on their own, your muscles squeeze them when you move, your breathing changes pressure in your chest, and outside pressure (like a massage, or water) pushes on your tissue. Take away those triggers, and lymph fluid slows down.
The fluid around your cells has exactly one way out and it doesn't leave by itself.
What Builds Up, and Why It Sticks Around
Small things move on their own just fine. Water, oxygen, sugar, and electrolytes are small enough to spread through fluid without much help.
Bigger things get stuck. Damaged proteins, inflammation signals, immune leftovers, and bits of broken-down tissue are all too large to move on their own. They only move when the fluid around them moves — and they leave only through the lymphatic system.
When fluid movement slows down, these leftovers just sit there. That's what congestion actually is. It's not water weight, and it's not a hydration issue. It's waste material that got delivered to your tissue and has no easy way to leave.
Why Congestion Steals Your Oxygen
Here's where the "executive workout" stops being a joke. Congestion actually costs your body oxygen, and the reason is simple.
Oxygen doesn't get pumped straight into your cells. It leaves the blood vessel and has to travel the rest of the way on its own. The farther it has to travel, the much longer it takes — travel time roughly quadruples every time the distance doubles.

What Heat Does to This Space

The Part That Actually Looks Like Exercise
Heat and cardio exercise share one specific signal — and it's worth being precise about what that signal is.
When blood moves fast through a vessel, it creates a dragging force along the vessel's inner wall. That force is called shear stress, and it tells the vessel wall to release nitric oxide, a molecule that keeps arteries flexible and working well. A long sauna session creates sustained shear stress in your skin's blood vessels — the same kind of signal that a moderate cardio workout produces.
And the results have actually been tested with a proper control group. In one study, eight weeks of regular heat exposure improved artery function, reduced artery stiffness, lowered blood pressure, and reduced thickening in the neck's main artery — with results similar in size to what exercise training produces. Another study found improved artery function in men with heart-disease risk factors after just two weeks of daily heat exposure.
But let's be clear about what heat doesn't do. It doesn't build bone strength. It doesn't build muscle. It doesn't create the cellular changes that come from real muscular effort. It doesn't improve balance or coordination. The executive workout is not a real workout, and no amount of science changes that.
What it is, accurately, is a heart-and-blood-vessel workout that requires zero physical effort. That makes it genuinely useful for patients who are too out-of-shape, too fatigued, or in too much pain to handle regular cardio yet. We often use heat as a stepping stone toward exercise, not a replacement for it. For everyone else, it's a good addition to a workout routine — and a poor substitute for one.
What the Second Round Adds
One round of heat does real work. It pushes fluid into your tissue, speeds up your lymphatic vessels near the skin, and gives your blood vessels a healthy stress signal. A single sauna session is not wasted time.
But here's the question: what does one round finish, and what does it leave undone? Heat pushes fluid into your tissue. That fluid can't go straight back into the blood vessel — it has to leave through the lymphatic system instead. If you just step out of the sauna and cool down slowly over the next half hour, some drainage happens. But it's slow, and there's not much force behind it.
A cold plunge changes both the speed and the force. It works through two separate mechanisms, and only one of them is actually about temperature.
The first is temperature itself. Cold makes the blood vessels near your skin tighten up fast, which drops the pressure in those vessels quickly — reversing the fluid flow in seconds instead of over half an hour.
The second has nothing to do with cold at all — it's pure physics. Water pressing on your body from the outside squeezes everything underneath and pushes fluid back toward your core.
This is why a cold plunge beats a cold shower. In one study, sitting in water at a neutral temperature — not even cold — still lowered heart rate by about 15% and blood pressure by 11–12%. Water pressure alone is doing real work here.
So a single round gives you a "fill" phase followed by slow, weak drainage. A round followed by a cold plunge gives you a "fill" phase followed by a fast, forceful drainage phase. Why Two Rounds — Not Five
Two rounds is a deliberate choice, not a random one. There's a case for it in both directions.
The case for a second round: you get a full second cycle, your blood vessels respond faster and stronger the second time, and each switch between hot and cold trains your heart-rate and blood-pressure reflexes in opposite directions. For patients whose nervous systems have been stuck in one gear for years — which describes a lot of our patients — those switches between hot and cold may matter as much as either temperature alone. There's also a practical upside: the cold plunge resets how hot you feel, so most people can handle more total heat time across two rounds than they could sitting through one long session.
The case against a third, fourth, or fifth round is less often made, but it matters. A review of heat studies across a dozen animal species found that hotter, longer, more frequent, and more closely spaced heat sessions all made a good outcome less likely. That's the typical pattern for this kind of stress response — helpful in the right dose, harmful past it. That's animal data, so we don't lean on it too hard, but it lines up with traditional Finnish sauna practice, which uses long rest periods between rounds instead of the fast-paced cycling that's become trendy.
The rest period isn't a break from the routine — it's part of it. Sitting in water actually raises pressure at the point where lymph fluid drains back into your veins, and higher pressure there makes drainage harder, not easier. The time between rounds is when that pressure settles back down and real drainage happens. Two rounds with real rest between them will beat five rounds with a minute of rest each.
The Benefit That Only Comes From Doing This Often
Everything so far happens within a single sauna session. There's a bigger, separate benefit that builds up over weeks of regular use.
Regular heat exposure increases your total blood volume by roughly 10%. In one study, plasma volume (the liquid part of blood) rose by 11.6%, and total blood volume rose by 4.5%. This happens because your body holds onto more protein and sodium than usual, and water follows them into your bloodstream. About two-thirds of this increase shows up within the first 24 hours, and the rest builds over the following weeks. The same study found slightly lower resting body temperature and a systolic blood pressure drop of about four points.
Why does this matter? More blood volume means more blood returning to your heart, a stronger heartbeat, and less strain on your nervous system when you stand up. If you get lightheaded when you stand, if your heart rate spikes for no clear reason, or if standing in a warm room or a long line makes you feel unusually bad — this is often the first thing to improve.
One result to expect, and not worry about: your hemoglobin and hematocrit numbers may come back lower on a follow-up blood test. This isn't anemia — it's dilution. You've added more plasma without losing red blood cells. In the study above, red blood cell volume stayed the same; the plasma just expanded around it.
This blood-volume benefit is built by doing this regularly, and it fades if you stop. That's the strongest reason to aim for four or more sauna sessions a week, instead of one long session on a Sunday afternoon.
Two Routines, Two Different Goals
Everything above makes the case for adding cold. But there's one goal where cold actually works against you — and getting this wrong will cancel out the benefit you were after.
If your goal is better sleep, don't end on cold.
The sleep benefit of a sauna doesn't come from being warm — it comes from your core body temperature dropping sharply after you get out. Heat widens your skin's blood vessels, so once you leave the sauna, you lose heat fast and your core temperature drops further and quicker than normal. That sharp drop is one of the strongest sleep signals your body has. Warming up before bed reliably helps you fall asleep faster and get more deep sleep.
A cold plunge cancels that out. Your blood vessels tighten, your body holds onto heat instead of losing it, and you get a jolt of alertness-boosting hormones instead. In fact, cold plunges roughly double your norepinephrine (an alertness hormone) within about two minutes.
So the same two tools split into two very different routines.

How to Get Started (and Who Shouldn't)
How often you go matters more than how long each session lasts. The long-term research on sauna use is based on sessions per week, not minutes per session. Someone who does 15 minutes, five times a week, will do better than someone who does 40 minutes once.
If you're new to this, don't jump straight into the full routine.
- Weeks 1–2: Heat only. 12–15 minutes, no cold at all.
- Weeks 3–4: Add one 60-second cold round. Start with water around 60°F instead of 54°F — it gives you nearly the same benefit with much less shock to your system.
- Week 5 and beyond: Work up to two full rounds. Only lower the water temperature once you're tolerating it well.
This routine isn't right for everyone. Never combine heat and cold exposure with alcohol.

Cold water causes a sharp spike in blood pressure, right on top of the blood pressure shift your body is already making as it leaves the heat.
